Shelter Liaison & Permanent Housing Facilitator

 

The service has been developed by Hedwig House, Inc. in collaboration with Montgomery Mental Health, Mental Retardation, and Alcohol Programs, to assist individuals and families with disabilities of mental illness, substance abuse, or dual diagnosis, currently living in or seeking admission to emergency shelters. The program will provide opportunity for rental assisted housing, to help participants move into permanent housing, along with linkages to treatment and rehabilitation. The program is funded by the Federal Government.

 

How The Program Can Help:

  1. It can provide consultation to shelter personnel, concerning special needs of disabled population.
  2. It can provide planning and linkages to treatment and rehabilitation services for participants- including case management, housing supports, skill training, and needs assessment.
  3. It can help participants move toward permanent housing. Rental subsidy available and other funding to help with the move and setting up permanent housing.
  4. Transportation available to help participants to get services and agencies.
